History:
A 42-year-old man presents to the emergency department with worsening left lower quadrant abdominal pain over the past four days. A contrast-enhanced CT scan of the abdomen and pelvis was performed.

Findings
There is a focus of inflammatory change in the left lower quadrant seen anteriorly to the descending colon. This is associated with a rounded area of fat attenuation surrounding the central vessel. These findings are most compatible with a diagnosis of focal acute epiploic appendagitis rather than diverticulitis, as clinically suspected. There is no fluid collection, ascites, or pneumoperitoneum.
