我们都知道流感和普通肺炎很容易和新冠发生叠加，接种流感疫苗和肺炎疫苗是低成本高收益预防这个疾病的方法，我们国家的流感疫苗接种率远远低于西方发达国家，这种差别的原因是什么？我们有没有可能通过什么办法提高接种率？
张文宏：流感疫苗接种率中国现在在2%左右，流感疫苗接种的成功率、保护率是60%左右。所以世界上要避免流感流行，达到群体免疫，基本上大家的接种率要达到90%以上才可以，因为保护率才60%，因为有病毒的变异。所以现在想通过流感疫苗的接种，基本上中国2%的水平达到每年抑制流感的流行一点可能性都没有。
中国2%的人接种了，80%的人是无所谓。怎么才会产生有所谓呢？每年冬天你都可能发一次烧，你有所谓吗？问题是你无所谓。如果发烧集中发生，造成了医疗挤兑，死了很多人，你就会有所谓了。所以到今天有没有所谓完全在于你的感受，你没有感受到有所谓，它的病死率是低的，但感染率非常高，每年全球因流感死亡的人数是30-60万，但这在70亿的人口里面你又觉得没什么，因为这个数据跟你没关系。
但是一旦有关系的时候会非常麻烦。所以在很多年前，大家看过一个公众号发的一篇文章，写的是流感下的北京中年，讲到一个老人家得了流感死掉了，老年人是流感的重点风险，所以今天接种流感疫苗不是为别人，是为你自己。所以今天在这里呼吁，建议重点人群老人和小孩还是要打疫苗。

We know that the flu and common pneumoniacan overlap COVID-19 very easily. An effective-cost approach with many benefitsto prevent these diseases is to get flu and pneumonia vaccines. Yet we see amuch lower flu inoculation rate in China than in developed countries. How isthat? Is there any way to increase this rate?
The flu inoculation rate in china is about2%. The flu vaccine has a 60% protection. If the world wants to contain fluoutbreaks as in terms of herd immunity, we need at least a flu inoculation rateof 90%. As we said flu vaccine only has 60% protection and we must considerabout the mutation as a variable. Given the above, an inoculation rate of 2% cannotcurb the flu outbreak at all.An inoculation rate of 2% means that about80% of people don’t think the flu matters. When does it matter? You might thinkit doesn’t matter to have a fever once in every winter. It doesn’t matter toyou. But if a majority are having a fever at the same time then this case canoverwhelm our medical system with a high death toll. You would think theopposite. Whether it matters depends on how you think about it. With a smallfatality rate, we don’t think it matters. But we must see the infection rate isquite high. Statistically every year about 300,000 to 600,000 die from the fluworldwide. When we compare this number to 7 billion people, you might get animpression that it’s irrelevant with your life.But when it is relevant, it’s gonna be bigtrouble. Years ago, there was an article written by an author whose elderlyfamily member was killed by the flu.. The elderly are most vulnerable to theflu. So to get the flu vaccine is not for others, it’s for yourself. I want tosuggest strongly that the most vulnerable people, the elderly and children, getthe flu vaccine.
