图一、入境深圳专属二维码，2022年3月4日更新

QR Code updated on March 4th,2022

       深圳入境旅客需在航班起飞48小时前完成入境深圳专项健康申报，以便深圳当地相关检疫部门核查，未申报旅客将无法从深圳入境。

       深圳入境申报除了扫描以上二维码填报以外，旅客也可以在购票后关注“中国南方航空”微信公众号，输入“深圳入境”，点击弹出链接填写相关信息完成申报。而官网及APP购票旅客将收到由南航发送的微信公众号推送信息。或手机短信，点击提示中的申报链接跳转至申报页面填写相关信息完成申报 。

    Every passenger who will enter Shenzhen must apply Shenzhen Entry Declaration 48 hours before the flight departure for verification by the relevant quarantine department by above QR Code. Passengers who have not declared will not be able to enter from Shenzhen.

      In addition, besides by scanning above QR Code to apply Entry Declaration, passengers could also search by our WeChat Official Account “中国南方航空”, input “深圳入境”, then click on pop-up link to fill the Entry Declaration. For passengers who bought tickets from APP or Website will accept the notice regarding Entry Declaration by WeChat Official Account or Text Messages, click on the link and fill the Entry Declaration.

入境深圳健康申报流程

1.填写中文姓名、证件号、手机号，点击去申报

Fill in your Name, Passport number, Phone number and click on declare.

2.按要求填写信息，注意此处中国籍旅客姓名一定要填写中文汉字姓名，ZHANG/SAN、ZHANGSAN等英文拼音均不合要求。

Please fill the information as required, for Chinese passengers please write your name in Chinese character, ZHANG/SAN、ZHANGSAN or other Pinyin are not required.

3.点击下一步填写航班信息，注意此处目的地详细地址需填写旅客在国内详细居住地，广东省深圳市、深圳宝安区、深圳xx隔离酒店均不合要求。

Click on next page and fill in your flight details, please fill the address with your detailed address residence in China. For example: Guangdong Province-Shenzhen, Shenzhen-BaoAn District, Shenzhen XX Quarantine Hotel are not required. 

4.点击下一步填写健康信息，提交完成申报。

Click on next page and fill your health condition, and submit.

IMPORTANT NOTICE：

       除了上述提前48小时或以上扫码“入境深圳专属二维码”（图一）按要求填报相关信息并保存好生成的报告备查以外，在雅加达机场完成值机手续后还需扫码“中国海关码”（图二）按要求填报相关信息后并保存好生成的二维码备查。雅加达-深圳航班旅客需要扫两次码，并保存二维码和报告结果备查。为保障顺利入境深圳，请各位旅客务必按相关要求落实。

    Besides applying “Shenzhen Entry Declaration” 48 hours before the flight departure (Picture 1) above, fill the information as required, and after successfully submitted kindly save or screenshot for future reference, in Jakarta International Airport passengers should also scan “China Customs Code” (Picture 2) after complete check-in procedure, fill the information as required, and after successfully submitted kindly save or screenshot for future reference. For Jakarta-Shenzhen flight, every passenger must scan two QR Code, and kindly save or screenshot with test result for future reference. In order to ensure entry, every passenger must follow relevant requirements.

       此外，绿码申领方面，南航CZ8354雅加达-深圳航班旅客在SOS MEDIKA KLINIK CIPETE和MEDISTRA HOSPITAL两家定点医院完成检测并获得PCR核酸检测阴性报告后，需要及时通过微信小程序“防疫健康码国际版”上传报告申请获得“HS”标识的绿色健康码（中国公民适用）或带“HDC”标识的健康状况声明书二维码（外国公民适用），才能在雅加达机场办理该航班的乘机手续。

     Passengers of China Southern Airlines CZ8354 Jakarta-Shenzhen flight who have completed the test at the two designated hospitals of SOS MEDIKA KLINIK CIPETE and MEDISTRA HOSPITAL and obtained PCR test negative report, they need to upload the report in time through the WeChat Mini Program "Epidemic Prevention Health Code" Only the green health code with the "HS" logo (for Chinese citizens) or the QR code with the "HDC" health declaration form (for foreign citizens) can be accepted for check in at Jakarta Airport.
