2、幼儿户口本复印件，户主页跟幼儿页
（复印在同一张纸上）
3、幼儿身份证复印件
4、父母身份证复印件及户口本复印件
5、幼儿转园证明或龙江医院体检报告
6、幼儿免疫证明（需凭打预防针本子到龙江医院防保科开具）
7、预防针本子复印幼儿信息页
8 、幼儿3R生活照一张及一寸彩照1张
