今天(10月9日)有媒体报道说,美国生物制药公司吉利德董事会主席兼首席执行官丹尼尔•欧戴(DanielO’Day)昨天发布公开信称,针对新冠病毒的药物瑞德西韦的三期试验数据已经在著名医学杂志《新英格兰》上公布。据介绍,三期临床实验数据来自美国国家过敏和传染病研究所开展的生物随机双盲安慰剂对照的三期研究,涵盖了全球约1060例住院患者。瑞德西韦三期临床数据显示,接受瑞德西韦治疗的新冠患者平均恢复时间比其他药物治疗的患者快了5天,而占研究总数85%的病情严重的患者恢复时间则快了7天;此外,瑞德西韦降低了患者发展到疾病更严重阶段的可能性,临床实验的最大一组病人,因病情严重而使用低流量氧气的患者,使用瑞德西韦之后降低了死亡率。据此,吉利德科学公司称,最新数据证明了瑞德西韦的有效性,这为抵御新冠病毒带来了前所未有的希望。


吉利德公司的上述成果,的确令人欣慰。在科学研究可以探微至人类DNA的时代,抵御新冠病毒、以对症药物及其治疗手段治愈新冠感染者,只是时间问题。但是,从此次新冠病毒的扩散速度以及传播力来看,对许多感染新冠病毒的人而言,时间就是生命。这也就是说,拖宕过久的药物研发,对相当一部分人没有意义。而吉利德公司之所以能够在第一时间给出成品药物进行抗病毒临床实验,其实也得益于其在SARS流行期间的对证药物研发投入。这看似“歪打正着”的投入为人类福祉带来的巨大增益,给科学研究、药物研制太多的启发。


10月5日,2020年诺贝尔生理学或医学奖被授予了在“发现丙型肝炎病毒”方面做出贡献的哈维•阿尔特(Harvey J. Alter)、迈克尔•霍顿(Michael Houghton)和查尔斯•M•赖斯(Charles M. Rice)。在全球新冠病毒疫情仍未被彻底遏制的背景下,今年的诺贝尔生理学或医学奖颁发给这些在抵御和治愈人类疾病方面做出贡献的科学家,可谓正逢其时。人们在感怀科学昌盛、感恩科学家的同时,当然希望针对新冠病毒的对证药物尽快问世。


上月下旬,权威医学期刊《柳叶刀》杂志刊登了加拿大不列颠哥伦比亚大学生命科学研究所所长约瑟夫•彭宁格(Josef Penninger)教授研发的新冠药物APN01(暂名)的实验数据。相关报道称,一名在ICU戴上呼吸机、命悬一线的45岁重症新冠患者,在服用APN01药物2天后,新冠病毒在肺部和血液中消失,患者康复出院……与吉利德的瑞德西韦一样,APN01的研发同样得益于SARS药物研发的经验。


在新冠病毒致全球超过3千万人染病、100多万人死亡且仍在蔓延的情况下,对证药物的研发与疫苗的研发是人类遏制新冠病毒的希望所在。



吉利德公布瑞德西韦三期临床数据:

患者平均恢复时间快5天

(低流量氧)患者死亡人数减少了70%


抗病毒药物瑞德西韦有了三期临床数据。

10月8日,吉利德科学官网发布了其董事会主席兼首席执行官Daniel O’Day的公开信,称瑞德西韦的最新数据发表在新英格兰杂志,这些数据来自美国国家过敏和传染病研究所(NIAID)领导的生物随机双盲安慰剂对照的3期研究,涵盖了全球约1060例住院患者。

吉利德方面称,最新的数据扩展了瑞德西韦的益处,并在与新冠病毒的斗争中提供了前所未有的希望。根据吉利德发布的消息,此次三期临床数据主要包括三方面的信息。

第一,接受瑞德西韦治疗的住院患者平均恢复时间快了五天,而患有严重疾病的患者则快了七天,而这些重病患者占研究总数的85%。

第二,瑞德西韦降低了患者发展到疾病更严重阶段的可能性,在这一阶段,患者需要额外的氧气支持。

第三,此次研究中最大的一组病人,即使用低流量氧气的患者,事后分析显示,瑞德西韦降低了其死亡率。在总人数中,结果也表明,死亡率有下降的趋势。

另外,在需要补充氧气的患者组中,接受瑞德西韦加标准护理的患者比接受安慰剂加标准护理的患者恢复快了一周。

在对接受低流量氧气治疗的患者的死亡率进行事后分析中,与安慰剂组相比,接受瑞德西韦治疗的患者死亡人数减少了70%,而使用低流量氧气的患者约占总患者人数的40%。

新冠疫情以来,除了预防性质的疫苗,瑞德西韦也被予以厚望。因为谐音一度被称为“人民的希望”,在美国,前不久确诊新冠肺炎的特朗普在治疗过程中也曾使用瑞德西韦。

在三期临床试验公布之前,瑞德西韦已经凭借前期研究数据在50多个国家/地区获得不同程度的批准或授权,用于治疗新冠肺炎。吉利德也就瑞德西韦的商业供应、定价等方面透露了一些消息。

这些数据包括4月29日,美国NIH发布的消息称,根据一项纳入1063名患者的随机对照试验的中期结果分析,接受瑞德西韦治疗的患者的康复时间比接受安慰剂的患者快32%。

吉利德也在当天发布了瑞德西韦治疗新冠肺炎的5天和10天临床试验结果,显示50%的患者的临床改善时间在5天治疗组中为10天,在10天治疗组中为11天。两个治疗组中超过一半的患者在第14天内出院。

公开信中提到在此次NIAID的整体研究结果中,看到了瑞德西韦更快恢复和减少疾病进展的益处。除了对患者的直接影响外,这些结果还显示,瑞德西韦对医疗系统的明显利益和价值,可以帮助减少医疗资源的使用,并减少患者住院的天数。

吉利德方面还表示,对于住院的新冠患者,不可低估将恢复速度提高五到七天的重要性。除了身体上的挑战外,每天与这种疾病有关的疾病还给处理分离和忧虑的患者及其家人带来情感上的损失。一些医生还提出了一个观点,即患者住院时间越长,继发感染的风险就越大。


欧盟签署框架合同

采购50万个疗程瑞德西韦


新华社布鲁塞尔电(记者李骥志 彭立军)欧盟委员会与美国制药公司吉利德科技8日签署联合采购框架合同,购买最多50万个疗程的瑞德西韦药物,同时保留采购更多药品的机会。


此次签署联合采购框架合同的包括27个欧盟国家以及挪威、冰岛、英国等共36个签署方。在采购框架下,所有签署方都可直接购买瑞德西韦。


欧盟委员会负责卫生和食品安全事务的委员基里亚基季斯说:“今天,我们为欧洲争取到了可供50万例病患使用瑞德西韦的权利。”她表示,欧盟将继续致力于提供安全、高效的新冠药物。


瑞德西韦是美国吉利德科技公司研发的一款新冠药物。此前,欧盟曾向吉利德科技购买33380个疗程的这款药物,以满足8月至10月间的用药需求。目前,这些药物已分批次配发至欧盟国家和英国。据称,8日新签署的采购合同可确保该药物不间断供应。

They had arrived in San Francisco, where their ship was decommissioned, and Paul bet that he would find himself a wife within two weeks. It would turn out to be a happy marriage, one that lasted until death parted them more than forty years later. Paul Reinhold Jobs had been raised on a dairy farm in Germantown, Wisconsin. Even though his father was an alcoholic and sometimes abusive, Paul ended up with a gentle and calm disposition under his leathery exterior. After dropping out of high school, he wandered through the Midwest picking up work as a mechanic until, at age nineteen, he joined the Coast Guard, even though he didn’t know how to swim. He was deployed on the USS General M. C. Meigs and spent much of the war ferrying troops to Italy for General Patton. His talent as a machinist and fireman earned him commendations, but he occasionally found himself in minor trouble and never rose above the rank of seaman. Clara was born in New Jersey, where her parents had landed after fleeing the Turks in Armenia, and they moved to the Mission District of San Francisco when she was a child. She had a secret that she rarely mentioned to anyone: She had been married before, but her husband had been killed in the war. So when she met Paul Jobs on that first date, she was primed to start a new life. Like many who lived through the war, they had experienced enough excitement that, when it was over, they desired simply to settle down, raise a family, and lead a less eventful life. They had little money, so they moved to Wisconsin and lived with Paul’s parents for a few years, then headed for Indiana, where he got a job as a machinist for International Harvester. His passion was tinkering with old cars, and he made money in his spare time buying, restoring, and selling them. Eventually he quit his day job to become a full-time used car salesman. Clara, however, loved San Francisco, and in 1952 she convinced her husband to move back there. They got an apartment in the Sunset District facing the Pacific, just south of Golden Gate Park, and he took a job working for a finance company as a “repo man,” picking the locks of cars whose owners hadn’t paid their loans and repossessing them. He also bought, repaired, and sold some of the cars, making a decent enough living in the process. There was, however, something missing in their lives. They wanted children, but Clara had suffered an ectopic pregnancy, in which the fertilized egg was implanted in a fallopian tube rather than the uterus, and she had been unable to have any. So by 1955, after nine years of marriage, they were looking to adopt a child. Like Paul Jobs, Joanne Schieble was from a rural Wisconsin family of German heritage. Her father, Arthur Schieble, had immigrated to the outskirts of Green Bay, where he and his wife owned a mink farm and dabbled successfully in various other businesses, including real estate and photoengraving. He was very strict, especially regarding his daughter’s relationships, and he had strongly disapproved of her first love, an artist who was not a Catholic. Thus it was no surprise that he threatened to cut Joanne off completely when, as a graduate student at the University of Wisconsin, she fell in love with Abdulfattah “John” Jandali, a Muslim teaching assistant from Syria. Jandali was the youngest of nine children in a prominent Syrian family. His father owned oil refineries and multiple other businesses, with large holdings in Damascus and Homs, and at one point pretty much controlled the price of wheat in the region. His mother, he later said, was a “traditional Muslim woman” who was a “conservative, obedient housewife.” Like the Schieble family, the Jandalis put a premium on education. Abdulfattah was sent to a Jesuit boarding school, even though he was Muslim, and he got an undergraduate degree at the American University in Beirut before entering the University of Wisconsin to pursue a doctoral degree in political science. In the summer of 1954, Joanne went with Abdulfattah to Syria. They spent two months in Homs, where she learned from his family to cook Syrian dishes. When they returned to Wisconsin she discovered that she was pregnant. They were both twenty-three, but they decided not to get married. Her father was dying at the time, and he had threatened to disown her if she wed Abdulfattah. Nor was abortion an easy option in a small Catholic community. So in early 1955, Joanne traveled to San Francisco, where she was taken into the care of a kindly doctor who sheltered unwed mothers, delivered their babies, and quietly arranged closed adoptions. Joanne had one requirement: Her child must be adopted by college graduates. So the doctor arranged for the baby to be placed with a lawyer and his wife. But when a boy was born—on February 24, 1955—the designated couple decided that they wanted a girl and backed out. Thus it was that the boy became the son not of a lawyer but of a high school dropout with a passion for mechanics and his salt-of-the-earth wife who was working as a bookkeeper. When Joanne found out that her baby had been placed with a couple who had not even graduated from high school, she refused to sign the adoption papers. The standoff lasted weeks, even after the baby had settled into the Jobs household. Eventually Joanne relented, with the stipulation that the couple promise—indeed sign a pledge—to fund a savings account to pay for the boy’s college education.
