Foreign Passenger Health Declaration Guidance
Dear Passenger:（Test Agent Name:Medistra Hospital,Jakarta）
Please follow the steps as below for health declaration if you are Foreign Nationality Passenger.
(Attention that the declaration only can be done after you get your own “Nucleic acid test report”
and “IGM Serum test report”)
1. Login the health declaration website by address:https://hrhk.cs.mfa.gov.cn/H5/
2. You can also Login the website via scan below QR code if you have wechat.
3. You need select “Apply for health declaration form” then register by your own e-mail after
login in the website.
4. Click on “Get the verification code” after fill in your e-mail address,and select “I have read
and agreed...” after fill in the e-mail verification code,then click on The next step.
5. Login in the website account with your register details.
6. Fill your personal information follow the steps of website.
7. You will get the health code with “HDC” logo after submit.
8. If you wanna check the code,just click on the “View and Show your health declaration
form”.Please make screenshot of the code,and the check in staff will check the photo.
