•Follow these simple steps to get your internationally recognized, bilingual, official Certificate of Vaccination.
Whether you plan to travel when the borders open or just want a bilingual record of your vaccination, you can apply for their International Certificate of Vaccination. Please note that regulations vary between cities so please check with your local authority to check.
Simply prepare the correct documents and take them to the International Travel Health Center in your city to apply for the little yellow book.
We recommend calling in advance to make sure your city offers this service and for any additional requirements.
The Vaccine certificate contains your photo, name and personal details and records of the dates and brand of the vaccine(s) you have received.

2 x ID Photos

Where to go?
To submit your application you need to visit the 国际旅行卫生保健中 (International Travel Health Center) in your city or province.
