这种可能比生孩子更痛的病，其实可以预防

隔壁办公室的张姐今年 52 岁，上个月刚当上外婆，给大家发完红鸡蛋后，整天忙得脚不沾地的。每天下班后，买菜做饭送餐，晚上还要陪夜。虽然很快人就瘦了一圈，但还很精神。

可是最近这半个月就不对了，一开始是左边脸上像火烧一样的痛，发作的时候满头大汗，用张姐的话来说就是，「当年生孩子的时候也没这么痛过」。没几天就冒出来一串亮晶晶的小水泡，旁边皮肤一片红。到医院一查，医生说是「带状疱疹」，开药之后，还一再嘱咐张姐暂时不要去看外孙了。

其实，张姐说的一点也不夸张。疼痛的程度可以用一些办法来量化，如视觉模拟评分法（VAS），用 0-10 的分数来进行大致的评估，0 分表示无痛，10 分表示最剧烈的疼痛。中国医学科学院的研究显示^[1]，分娩痛的平均分是 8.5，而疱疹后的神经痛平均分为 6.5。德国有研究显示^[2]，带状疱疹神经痛平均分为 6-7 分，部分人可以达到 8-10 分。

考虑到个体差异的存在，对于有些人来说，「带状疱疹」的确有可能比生孩子更痛。而且，生孩子痛在一时，带状疱疹后神经痛没完没了，让人绝望，是导致 70 岁以上慢性疼痛患者自杀的主要因素^[28]。

而且，这种疾病还会挑中老年人「欺负」，随着年龄增大，发病率也会增加。所有带状疱疹患者中 50 岁以上的占了一半。50 岁以下的每年发病率不到 3‰，而 80 岁以上的就接近 11‰ 了。普通人的终生患病风险约为 30%，而 85 岁以上未接种疫苗的人，有一半以上会受到该病的影响^[3~5]。

▍带状疱疹是种什么病

带状疱疹是由于感染了一种病毒而致病的，这种病毒是水痘-带状疱疹病毒。人体在第一次感染这种病毒时，疾病为水痘。水痘治好以后，有些病毒可能成为漏网之鱼，偷偷地潜伏在神经系统的某些角落。等到人体出现免疫力下降的时候（如衰老、劳累、生病等），它就可能趁机再次作乱，这次的疾病为带状疱疹。

因为皮疹和水疱随着神经的走向而分布，常常呈条带状，所以它的名称中才有「带状」二字。它也有一些俗称，如蛇盘疮、生蛇、皮蛇等。

这两种疾病的共同点是都具有传染性，所以医生才嘱咐张姐不要去看外孙。但它们也有很多不同，水痘会造成明显的瘙痒，而带状疱疹则常常带来难以忍受的疼痛和其他一些并发症。

带状疱疹最常见的并发症是带状疱疹后神经痛,眼部并发症也比较常见的。当病毒感染三叉神经的眼支时，可能会发生眼球并发症。如果不及时治疗，感染可能会导致结膜炎、角膜炎、角膜溃疡、虹膜睫状体炎、青光眼和失明。其他并发症包括细菌性超级感染、周围神经麻痹、感觉丧失、脑炎等^[6~9]。

▍带状疱疹如何诊治

怀疑患有带状疱疹时，要及时就医，除了能缓解症状、降低传染性，还能减少并发症的发生。一旦出现带状疱疹后神经痛，可能就会面临 3 个月以上，甚至很多年的痛苦煎熬。

就诊时，先到皮肤科，有经验的医生常常一望便知。如果难以鉴别，也可以通过 PCR 检测病毒 DNA 等技术手段来协助诊断。

对于已经出现的疼痛，可以采用一些药物来缓解。外用的有利多卡因、辣椒素等，全身用药可以考虑：布洛芬、吗啡、卡马西平、阿米替林、加巴喷丁或普瑞巴林等，必要时也可以考虑加用激素类药物^[10~16]。但这些都称不上「特效药」，只能缓解症状，不能保证「药到病除」。

要对付带状疱疹，最好的办法是提前预防，减少发病。最好的预防工具就是疫苗。

▍带状疱疹疫苗

目前世界上已经有两种带状疱疹疫苗。一种是默克（Merck）公司生产的减毒活疫苗 Zostavax，2006 年在美国率先上市^[17]；另一种是葛兰素史克（GSK）公司生产的重组亚单位（灭活）疫苗 Shingrix，2017 年在美国上市^[18]。

这两种疫苗都能有效降低带状疱疹的发病率：Zostavax 平均有效率为 51.3%，Shingrix 为 96.2%。但疗效都会随年龄的增长而降低^[19~21]。针对带状疱疹后神经痛，Zostavax 可降低 66.5%，Shingrix 则能降低 91.2%^[22~23]。可以看到，Shingrix 的预防效果明显优于 Zostavax。

因为疫苗的性质不同，减毒活疫苗 Zostavax 不能用于存在免疫缺陷的人群，也不能用于孕妇和儿童等。而重组亚单位（灭活）疫苗 Shingrix 的安全性相对大一些，加上预防效果也更好，因此获得了美国免疫实践咨询委员会（ACIP）的大力推荐：建议所有 50 岁以上的健康成年人都应接种带状疱疹疫苗，最好是接种 Shingrix；建议之前曾接种过 Zostavax 疫苗的患者今后考虑接种 Shingrix ^[24~26]。

2019 年 5 月，重组带状疱疹疫苗 Shingrix 在我国获批上市，中文名叫「欣安立适」^[27]。中国每年约有 300 万成年人受带状疱疹影响，他们将获得前所未有的预防机会，远离病痛。
